Terms of Service

Effective date: May 24, 2026

These Terms of Service ("Terms") govern your use of the website at southlakedental.com (the "Site"), operated by Southlake Dental ("we," "us," or "our"). By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site. These Terms apply to the Site only; they do not govern the dental treatment we provide, which is subject to separate consent and financial agreements.

1. The Site is informational, not medical or dental advice

The Site provides general information about our practice and services. Nothing on it is a substitute for professional dental or medical advice, diagnosis, or treatment. Always consult a qualified provider, and never disregard or delay professional advice because of something you read here.

2. No doctor–patient relationship is created through the Site

Using the Site, submitting a form, does not create a doctor–patient relationship. That relationship begins only when you become an established patient through an in-office visit and intake.

3. Appointment requests are not confirmed appointments

Submitting a request, clicking "Book Now," is a request for us to contact you, not a confirmed booking. An appointment is confirmed only when our team arranges a specific date and time. We aim to respond within one business day.

For urgent or emergency dental issues, do not wait for a Site response — call us at (817) 481-6553, or call 911 or go to the nearest emergency room for a medical emergency.

4. Third-party booking platforms

Clicking "Book Now" may direct you to a separate third-party scheduling platform (Yapi). Your use of these platforms is governed by their terms and privacy policies, not ours. We do not control them and are not responsible for them.

5. Acceptable use

You agree not to: submit false or fraudulent information; send spam, automated requests, or other abuse; attempt unauthorized access to the Site or related systems or interfere with its operation; use the Site or its content commercially without written permission; or use the Site unlawfully.

6. Intellectual property

The Site and its content (design, text, graphics, logos, images) are owned by Southlake Dental or used with permission and protected by U.S. and international copyright and trademark law. You may view, download, and print pages for personal, non-commercial use only, and may not otherwise reproduce, modify, distribute, or publicly display Site content without our prior written permission, except as permitted by law.

7. Patient testimonials and reviews

Testimonials reflect individual patient experiences. Results vary, and testimonials are not a guarantee of any outcome.

8. External links

The Site may link to third-party websites we do not control. We are not responsible for their content or practices, and a link does not imply endorsement.

9. Disclaimer of warranties

The Site is provided "as is" and "as available," without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant the Site will be uninterrupted, timely, error-free, or free of harmful components.

10. Limitation of liability

To the maximum extent permitted by law, Southlake Dental and its owners, staff, and affiliates will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of the Site, even if advised of the possibility. Any liability that cannot be excluded is limited to the maximum extent permitted by law. Nothing here excludes liability that cannot lawfully be excluded.

11. Indemnification

You agree to indemnify and hold harmless Southlake Dental and its team from claims, damages, or expenses (including reasonable attorneys' fees) arising from your misuse of the Site or violation of these Terms.

12. Governing law and venue

These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules. Any dispute will be brought exclusively in the state or federal courts located in Tarrant County, Texas, and you consent to their jurisdiction.

13. Changes to these Terms

We may revise these Terms from time to time, posting the updated version here with a new effective date. Continued use after changes are posted means you accept them.

14. Severability

If any provision is found unenforceable, the rest remain in full effect.

15. Contact

Note: These Terms are provided as a general-purpose template for a dental practice website. They are not legal advice. Have your attorney review and adapt them to your specific business practices and jurisdiction before relying on them in production.
